The Role:
We are seeking for an experienced and enthusiastic Support Coordinator with strong case management skills and an excellent understanding of NDIS support coordination and mental health.
In this role, you will be responsible for managing and coordinating a range of supports for NDIS participants, ensuring that they receive personalised, goal-oriented services.
Your ability to build strong relationships and effectively navigate the NDIS framework will be critical to your success.
Key Responsibilities:
Build and maintain strong relationships with participants, families, service providers, and stakeholders.
Developing and implementing tailored support plans and manage a diverse caseload of NDIS participants.
Monitor and review participant goals and service agreements to ensure quality outcomes.
Maintain accurate and up-to-date case notes and compliance documentation.
Advocate for participants to access appropriate supports and resolve service delivery challenges.
Coordinate a variety of services, including community participation, capacity building, and in-home supports.
Stay current with NDIS policies, funding changes, and best practices.
What We're Looking For:
Proven experience as a Support Coordinator.
Strong understanding of the NDIS framework, including funding plans and service agreements.
Excellent case management, organisational, and time-management skills.
Exceptional communication, advocacy, and interpersonal skills.
Proficiency in Microsoft Office suite and using participant management related software.
Relevant qualifications in Social Work, Disability Services, Mental Health, or related fields (preferred).
Current driver's license and willingness to travel as required.
Valid NDIS Worker Screening Check and Working with Children Check (or willingness to obtain).
