As a Care Coordinator you will provide complex care coordination in the rostering of services for clients using our Client Management System with the aim of assisting people to live independently in their homes.
You will work closely with Case Managers, clients, carers and external services in the provision of appropriate, targeted and quality services to our clients.

Key Responsibilities To provide complex care coordination to clients across various funding streamsTo develop positive working relationships with Case Managers to provide streamlined high quality service delivery which is person centred and reablement focussedTo be responsible for data entry and care coordination activitiesTo build positive relationships with Support Workers to promote person centred and reablement focussed service deliveryTo develop partnerships with clients to ensure proactive solution focused supportWhat you bring Current Drivers Licence essentialCertificate IV in Community Services or a related fieldExperience in Aged Care, Disability or Community CareExperience with people with DementiaPrevious experience in care coordinationKnowledge and understanding of person centred service deliveryAbout Calvary We are a leading provider of quality, integrated care that puts our patients, residents and clients at the centre of everything we do.
