The Exciting Opportunity: Lead and mentor a team of Assessors delivering aged care assessments across WAOversee and coordinate service delivery to meet operational goals and KPIsUse a wellness and reablement approach to promote independence for older adultsProvide cultural sensitivity in assessments for diverse groups, including ATSI and CALD communitiesContribute to the development and expansion of aged care services in WAMaintain a caseload, performing assessments and triage when needed What Makes This Great: Flexible work arrangements: hybrid model with WFH options and autonomyA strong and supportive team culture, recognised as sector-leadingCompetitive salary with salary packaging and other employment benefitsOpportunities for professional growth through ongoing training and developmentA focus on work-life balanceBe guided and supported by your dedicated 3D Recruit Career Consultant, assisting with CV creation, interview preparation and career advice$110k - $120k + super About You: A Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker with unrestricted registration (AHPRA or professional association)Proven experience conducting aged care assessments in residential or community settingsLeadership experience within aged careStrong communication and interpersonal skills, with a client-centred approachPassion for promoting independence and well-being among older adultsAbility to lead teams, manage competing priorities, and meet operational objectivesFlexibility and a willingness to undertake triage or caseload duties as required What Next?
