DescriptionRole Purpose The Team Leader, Program Delivery Services is responsible for the design, development, delivery and viability of a range of innovative groups and programs for Aged Care and NDIS participants.
The Team Leader will provide opportunity for participation in a range of innovative programs that will be developed and delivered in response to customers' interests, plans and goals aimed at supporting their health, wellbeing, independence, socialisation and engagement with the community.
The Team Leader will develop and implement strategies to grow and expand group service offerings with increased customer participation.
The Team Leader will manage a team of direct reports and provide supervision and escalation for a team of Community Support Workers.
Key Outcomes / Accountabilities Design, develop, source and implement a range of innovative programs and customer focused activities that support customer goals, needs and plans and foster social and community connections.Collaborate and engage with internal and external stakeholders to identify programs and group services that are culturally appropriate, inclusive, and adaptable to varying support needs.Identify and develop partnerships with external providers for the provision of specialist groups and programs.Ensure all programs and participants are assessed for suitability, quality, safety and acceptable risk tolerances.Work collaboratively with others to develop and publish a range of promotional materials including program calendars, activity flyers and brochures including social media campaigns.Program Assessment & Review Continuously assess program effectiveness through customer and carer surveys, feedback, and data analysis.Identify and track trends in customer engagement and satisfaction to inform improvements and identify new initiatives.Regularly review programs to ensure activities align with contractual obligations, risk tolerances, statutory and legislative requirements and organisational standards.Budget & Resource Management Develop comprehensive program budgets ensuring financial viability through the coordination of appropriate levels of customer participation and resourcing in line with funding streams across HCP, CHSP and NDIS and ensure programs are delivered within budget with resourcing monitored and adjusted to ensure viability.Provide effective and timely team leadership, support and supervision to the Program Delivery Team including capacity building, coaching, feedback and performance development.Quality & Compliance Ensure the operations, functions and responsibilities of the Program Delivery Team are delivered in accordance with Aged Care and NDIS Quality and Safety Standards, Program Guidelines, Lifebridge policies and procedures and future state reform agendas and participate in the development of program delivery related policies, procedures, protocols and processes.Business Growth & Strategy Develop and implement a range of strategies to grow aged care and NDIS customer participation in Lifebridge groups, programs and activities.Identify and develop partnerships with external providers for the provision of specialist groups and programs.Actively participate in industry forums and develop networks to maintain expertise in Aged Care and NDIS standards, reforms and program related service delivery opportunities.Minimum Requirements Tertiary qualifications in Aged Care, Disability and or Community Services or equivalent.Strong knowledge of the Aged Care Standards, NDIS Quality and Safeguarding Framework and associated funding and pricing requirements.Demonstrated experience in the development and delivery of innovative community-based programs within aged care and or NDIS services.Demonstrated experience leading a team and achieving performance outcomes.Advanced verbal and written communication skills.Advanced knowledge and application of Office 365 suite and various customer management systems.Advanced customer service, relationship building and relationship management skills.Sound financial, budgeting and analysis skills.Effective time management, organisation, problem solving and conflict resolution skills.NDIS Worker Check (or ability to obtain one).Current driver's license and roadworthy vehicle. #J-18808-Ljbffr
