Join Our Team as a Coordination Lead (Transition Care Community)We are thrilled to announce an exciting opportunity for a Coordination Lead (Transition Care Community) to become part of our Brightwater at Home team on a permanent, full-time basis.
This role is pivotal in supporting a team of Transition Care Community coordinators and administration assistants as they manage the daily operations of our Transition Care Community Services.As the Coordination Lead, you will oversee the development and implementation of efficient, client-focused care plans and coordinate a range of services for our Transition Care clients transitioning from hospital to community.
You'll work collaboratively with the Aged Care Hub, major tertiary, district, and private hospitals, as well as Transition Care Placement, to streamline procedures that reduce admission wait times and bed blockages, all while enhancing the client's journey back to their own home.Benefits of working for Brightwater:A fully maintained tool of the trade motor vehicleThe ability to salary package – meaning you can legitimately pay less tax and increase your take-home payOur unique staff benefits program, including access to BrightwaterPlus which gives you discounts with over 370 retailersPrivate health benefits and discountsExtensive learning and development opportunitiesTo be suitable for this role, you must demonstrate:A tertiary qualification in Social WorkManagement and leadership experience in a fast-paced, complex healthcare environmentThe ability to lead, motivate, and coach team members, including care workers, coordinators, administration assistants, schedulers, and trained professionals such as Allied Health Professionals and NursesProven experience in managing admissions and discharges within a healthcare or social services settingA strong understanding of key performance indicators and the ability to use data to drive improvementsExperience in providing effective administrative and business supportProficiency in allocating resources and services within budget and required timeframesPrevious experience in coordination or case management, ideally in aged care or health-related servicesExcellent computer skills with a solid knowledge of Microsoft applications, including Outlook, Word, Excel, and TeamsStrong written, verbal, and interpersonal communication skills, with the ability to build rapport and establish successful working relationships with stakeholdersExceptional customer service skills and a clear understanding of privacy and confidentiality requirementsA demonstrated ability to work independently using initiative and critical thinking, while also collaborating inclusively as part of a teamFlexibility to manage multiple requirements simultaneously, organize and prioritize tasks effectively, problem solve, and meet deadlinesA current Western Australian 'C' Class Driver's Licence (desirable)A satisfactory National Police Certificate obtained within the last 12 monthsAbout Brightwater:At Brightwater, we welcome people with the full diversity of life experiences, thoughts, and beliefs.
