Southern Cross Care QLD has an exciting opportunity for a proactive and self-directed full-time Care Partner to join our Community Team working across Miles, Tara, Chinchilla, Meandarra, and Jandowae Community Care area.
As our new Care Partner, you will be a champion for our Aged Care clients living at home, ensuring great service delivery and top-notch care for those we serve.

As a Care Partner for the Western Downs, you will work collaboratively with our elderly clients, who live around Miles, Tara, Chinchilla, Meandarra, and Jandowae area to engage in services that enable them to remain living in their home.
Based at one of our Home Care Offices in the area, in this dynamic role you'll manage a caseload of Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) clients, driving towards exceptional care outcomes.
Your expertise will ensure that each client's needs are met with top-quality service and support.
You'll be the key player in coordinating care strategies and using your in-depth knowledge and skills to make a real difference in our clients' lives.
Key Responsibilities Will Include Assessing the needs and preferences of each client in a caseload Developing personalised care plans in collaboration with clients and their families Coordinating the delivery of services such as personal care, meal preparation, cleaning, and transportation Monitoring the implementation of care plans and adjusting as needed Ensuring CHSP services are provided in accordance with CHSP guidelines Advocating for clients' rights and preferences Conducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectively Providing information and support to clients and their families regarding available services and resources Liaising with healthcare professionals, service providers, and community organisations to ensure coordinated care What will you bring to the role?
Experience in a similar role within Aged Care or Disability Services Sector Qualifications in Nursing or Allied Health (desired not essential) Demonstrated knowledge and experience in delivering services under Home Care Package Program Demonstrated knowledge and experience in delivering services under the Commonwealth Home Support Program (CHSP) Demonstrated knowledge and experience of working in accordance with Aged Care Standards and legislation Ability to manage time efficiently and prioritise responsibilities Excellent communication skills.
Ability to communicate clearly and effectively with team members and clients Exceptional interpersonal skills and the ability to relate to a variety of people Who is Southern Cross Care Queensland?
Southern Cross Care Queensland (SCCQ) is a Catholic not-for-profit organisation established by the Knights of the Southern Cross, more than 40 years ago.
They saw a need to provide compassionate care and inclusive service to some of the most vulnerable, the ageing population, in the community.
Since then, SCCQ has grown to employ over 1700 staff caring for and supporting more than 2000 people across 13 residential aged care homes, 5 retirement villages, 28 social houses, the National Disability and Insurance Scheme (NDIS) and home care services in metropolitan and rural Queensland.
