Our focus is on improving health care experiences and championing greater access, choice, and control for people in Australia when it comes to managing their health.Core to the Amplar Health's strategy is substantial growth in our rehabilitation in the home, hospital substitution, hospital avoidance and clinical home care delivery.Your OpportunityAs a TCP Coordinator (RN) you will be responsible for supporting our Transition Care Program and Home Care Package consumers in South Australia to maximise their independence and wellbeing.
This amazing position will see you visit consumers' homes in the community to undertake holistic conversations and assessments with consumers and their representatives to create comprehensive care plans with them that support their reablement goals and care requirements.This role will also assist with case coordination of appropriate TCP consumers:Maximise their ability to achieve their program goalsCoordinate complex and high acuity person-centred care to improve consumers' physical and/or cognitive functioningLiaise with external and internal providers as required for service transitionParticipate in regular Multi-D meetings with internal and external stakeholdersLiaise with the relevant TCP metro LHN teams to ensure coordinated service deliveryWhat will set you apart from the rest?Working in this autonomous position you will have a passion for working within the community aged care setting, enjoy goal-oriented reablement and creating individualised care planning.

You will be consumer and goal-focused whose enthusiasm for this area of work is driven to engage with and empower our consumers to achieve positive outcomes together with:Recent nursing experience supporting consumers within the community settingExperience working within a multi-disciplinary teamExperience supporting consumers within the community settingExperience within the home care package/transitional care package setting is highly desirableExceptional communication and customer service skills and the ability to build a rapport with consumers and external agenciesAbility to work autonomously but contribute to a supportive and engaging teamStrong administration skills and the ability to 'think outside the square'This role will initially be offered as 0.4 FTE (2 days a week) with the option to increase hours in the near future.
We offer a Monday to Friday roster (school hours friendly); the successful applicant will be required to work every Wednesday and able to select the other day you prefer to work.
We promote flexible working arrangements in our team and support work from home for part of the working week.The majority of the work will be located in the south of Adelaide, so willingness to travel is a requirement of the role.
