Right at Home provides exceptional in-home care and support for a range of clients in the Perth South-Eastern Suburbs, and our mission is to improve the quality of life for those we serve.Due to continued growth of our business, we urgently seek a dynamic individual to take on a newly created role of Relationship Officer responsible for raising awareness of our organisation across Perth and the South-Eastern suburbs of Western Australia, specifically, to grow the Perth region.The Relationship Officer needs to demonstrate the ability to create digital marketing and promotional material across various social media platforms to raise awareness across the community and home care sectors and possess strong stakeholder management skills to build strong partnerships with these groups long-term.You will report directly to an amazing Client Liaison Officer who is self-driven and committed to offering the highest level of service to clients we currently serve.Your role will be to develop new business opportunities and you ideally should have:Experience in the Community or Home Care sector (advantageous but not essential)The ability to promote services and programs to external stakeholders across the health and human services sectors.We deliver services under a range of funding sources, including private pay clients and My Aged Care, with our primary responsibility to provide quality home care and assistance to our fast-growing client base.The successful applicant will be working with our care team, liaising with clients, family members, staff, and a wide range of stakeholders to ensure services are provided in accordance with client needs and the goals and objectives of the business.Additionally, completing assessments, reviewing client plans to ensure we deliver quality outcomes for all, and managing internal and external sales enquiries to develop and maintain new and existing referral sources.If you are outgoing, self-driven, motivated, and a self-starter and strive to deliver the highest level of service, then we urge you to apply for this newly created role and make this your own.QualificationsThe Relationship Officer will be responsible for:Building strong connections with clients and their familiesService coordinationOrganising support to clients with disabilities and monitoring and reporting on their progressReferring clients to agencies that can provide additional helpAssessing client needs and resources for health, welfare, and other facilities and servicesResponding promptly to emails, enquiries, and requests from clients, staff, and managementMaintaining accurate records of incidents and feedback as requiredRe-organising work priorities to meet the needs of a fast-paced environmentSupporting all clients to meet their goalsConducting Client Care Assessments and Care PlanningSupervision and management of Care StaffDiscussing and promoting Right at Home's services with medical professionals and referral sourcesPerth based with some travelAbility to work in a fast-paced environmentPreferred SkillsExperienced in assessing and planning client needsSound understanding of the Aged Care and Disability systems is idealA strong client focus, which is based on their goals, preferences, and needsDemonstrated experience managing staffCurrent National Police CheckWorking with Children CheckFull Driver's LicenceAll candidates must have the right to work in AustraliaOrganisational, administrative, and time management skills are essential
