As a Level 3 Social Worker, you'll be part of the pioneering Specialist Intellectual Disability Health Team (SIDHT) at Western Sydney Local Health District (WSLHD).
This multidisciplinary outpatient service is committed to enhancing health outcomes for individuals with intellectual disabilities and complex health needs.

Where you'll be working Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Cumberland and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region.

Your Role As part of this dynamic team, you will: Deliver time-limited, client-centred services, including assessment, care planning, and navigation for young people and adults with intellectual disabilities.
Enhance the capacity of primary healthcare providers and mainstream services to meet the needs of this population.
Address health inequities and support access to specialized services.
Lead quality improvement initiatives and contribute to the service's ongoing development.
About You We are seeking a compassionate and skilled professional who:
Holds a tertiary qualification in Social Work and is eligible for membership with the Australian Association of Social Workers (AASW).
Has extensive experience in providing evidence-based care, particularly in complex and critical settings.
Demonstrates advanced clinical reasoning and a commitment to multidisciplinary collaboration.
Excels in communication and embraces continuous learning and improvement.
Understands the principles of intellectual disability-informed care and their practical application.

Showcase your experience and how you meet the essential qualifications for this rewarding role.Essential Criteria Qualification: Tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
Registration in line with Award requirements: Membership of the AASW.
Selection Criteria Relevant qualifications as a Social worker and eligible for registration with the Australian Association of Social Workers (AASW).
Extensive post graduate clinical experience providing evidence-based assessment and treatment for consumers and their families.
Ability to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
Demonstrated ability to work effectively within a multidisciplinary team to accomplish service goals and improved consumer care and outcomes.
Highly developed written and verbal communication skills and competence in the use of information technology systems.Ability to initiate, lead, complete and share quality improvement initiatives and services evaluation processes and lead ongoing quality improvement activities with other staff.
Extensive contemporary experience and knowledge of social work practice, as well as demonstrated understanding of the principles of intellectual disability informed care and its applications when working with consumers and families/carers.
