Employment Type: Permanent Part TimePosition Classification: Social Worker Level 3Remuneration: $54.76 - $56.54 per hourHours Per Week: 20Location: Westmead Hospital Part time permanent senior social worker position within a well-supported multi-disciplinary team based in an inpatient mental health unit, Westmead Hospital.
The applicant should have at least three years' experience working as a social worker in the community or inpatient setting. Where you'll be workingWestern Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region.

What you'll be doing As a Social Worker you will support WSLHD Older Peoples Mental Health Service by providing high-quality social work assessments and interventions for older patients in the acute inpatient unit.
In this role you will also be responsible for:
Providing evidence-based social work assessments and interventions.Conducting comprehensive mental health assessments, care planning, and evaluation.Assisting consumers and carers with complex and severe mental illness and aging issues.Delivering high-level services throughout the consumer's recovery journey.Leading discharge planning and community reintegration.How to ApplyApply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date. Essential Requirements: Qualifications: Tertiary qualifications in Social WorkRegistration: Eligibility for Australian Association of Social Work (AASW) membership.Selection Criteria: Substantial experience and skills in Social Work assessment, planning and evaluating interventions for older consumers with complex mental health conditions and comorbidities.Ability to work independently and as part of a multidisciplinary team.Demonstrated working knowledge of NSW Mental Health Act 2007 and NSW Trustees and Guardian Act 2009.Demonstrated advanced clinical knowledge, skill, and significant clinical experience in professional and clinical supervision, and support of staff.Demonstrated ability to independently problem solve, apply professional and ethical boundaries when working with complex situations as part of a multidisciplinary team.Demonstrated ability to initiate, lead, complete and evaluate quality improvement initiatives and/or research focused on patient/consumer outcomes.Class C Driver's License.Requisition ID: REQ532753Applications Close: 5th December 2024
