Where you'll be working The WSLHD Dementia Advisory Service operates five days a week from Westmead Hospital. You'll job share with a colleague, working two days a week.
Deliver tertiary care for the Greater Western Region.Align with our vision for better health services.Uphold values of collaboration, openness, respect, and empowerment.Be part of a dedicated team focused on enhancing dementia care.What you'll be doing The Dementia Advisory Service (DAS), WSLHD, is funded by the Commonwealth Health Support program (CHSP). The primary objective of this community-based service is to help people with Dementia and their carers through:
Early intervention case workEducation and Support Groups for patients and carersAppropriate referrals to services and supportsWork autonomously with advanced clinical expertise in dementia careDeliver high-quality services to clients, families, and carers in the communityPartner with service providers and lead community development initiativesHow to Apply Apply online by submitting your resume, addressing the criteria listed below by providing examples that demonstrate your ability before the closing date.
Essential Criteria: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
Selection Criteria: Bachelor of Social Work and eligibility for membership of the Australian Association of Social Work (AASW), and a current Driver's licenceDemonstrated clinical experience in Dementia careExperience in community development and working in partnership with service providersDemonstrated experience in the provision of staff and/or student supervisionExperience in developing and implementing policies and procedures for service improvement and skills in providing training and educationWell-developed time management skills with excellent written and verbal communication, including report writing skillsDemonstrated ability to work with minimal supervision and as part of a multidisciplinary teamDemonstrated experience in quality assurance and/or clinical practice improvement activitiesRequisition ID: REQ534782 Applications Close: 08th December 2024 Need more information?
