Job Summary The Adult Cardiothoracic Surgery Clinical Nurse Coordinator is a nurse who, because of advanced education, in-depth clinical knowledge, and experience in cardiothoracic surgery, possesses the skills necessary to provide expert coordination of care for patients with complex heart diseases, including those undergoing cardiothoracic surgery for coronary, valve, and aortic disorders.
The specialized knowledge and advanced skills required for this role include clinical expertise in managing the critical care patient population, evidence-based practice (EBP), education and mentorship, consultation, performance improvement, and team leadership.
Under the general direction of the Director of Heart & Vascular Inpatient Services and the Chief of CT Surgery, the primary responsibility of the Adult Cardiothoracic Surgery Clinical Nurse Coordinator is the management and coordination of high-quality pre and post-operative care for patients undergoing cardiothoracic surgical procedures in the Moffit / Long Hospital and ORs, as well as patients receiving medical management in the outpatient clinic setting.
This role emphasizes collaborative management, education, and continuity of patient care throughout the entire episode of care, including pre-admission, hospitalization, discharge, and follow-up.
The Adult Cardiothoracic Surgery Clinical Nurse Coordinator will triage patient communication and work with the team to meet the needs of this patient population.
They will monitor processes, therapeutic benefits, and side effects of multiple therapies, facilitating communication among faculty, Nurse Practitioners, Physician Assistants, house staff, Patient Coordinators, referring physicians / offices, support services, external agencies, patients, families, and patient care staff regarding hospitalized patients who will be discharged to maintain continuity of follow-up care.
Required Qualifications Five (5) years of clinical employment within the past seven (7) years, preferably in Vascular Surgery.Recent clinical outpatient experience, preferably in Adult Vascular Surgery, with in-depth knowledge of one or more clinical specialty areas, including diagnoses, treatments, standards of practice, and patient / family educational consulting and resources.Advanced knowledge of methodology for conducting evidence-based research, data collection, analysis, and reporting.Advanced critical thinking and problem-solving skills to assess highly complex issues and develop multiple options for resolution.Development and implementation experience with peri-operative and inpatient clinical and ERAS pathways for the Cardiothoracic Surgery Patient.Demonstrated ability to use outcomes data to formulate and implement quality initiatives.Advanced interpersonal skills, with the ability to provide education, training, and direction to nursing staff, and to develop positive working relationships with patients, families, employees, and outside agencies as required.Advanced skills in written and verbal communications, with the ability to convey complex and / or sensitive information in a clear and concise manner.Preferred Qualifications Registered Nurse in the State of California.Current California licensure as an RN / NP, RN / CNS, CRNA, MHA MBA, or DNP, Master of Science in Nursing.Certified Clinical Nurse Specialist, Master of Science: Nursing, Nurse Practitioner (FNP, ANP, ACNP, or AGACNP) or licensure / certification in area of specialty.AHA CPR / BLS.ACLS.About UCSF At UCSF Health, our mission of innovative patient care, advanced technology, and pioneering research is redefining what's possible for the patients we serve – a promise we share with the professionals who make up our team.
