We are seeking a dedicated and caring person to join our team at Anglicare's Donald Robinson Village - Kirrawee , to provide a high level of customer service to residents and their families, to build rapport with residents and support them to age in place, and to also assist in welcoming and onboarding new residents into village life.

Reporting to the Village Manager, the role will suit a person who understands the care needs of older people and has a comprehensive knowledge of community aged care options and navigating My Aged Care.
The role supports residents in accessing care and support when they need it, with a focus on health, wellbeing, and social needs.
Key responsibilities:
Undertake health and wellbeing assessments for new and existing residents.
Build rapport with residents to identify Home Care services that best meet their care needs.
Support residents through the My Aged Care process to obtain assessments and referral codes to access home care services.
This includes referrals and advocacy as required.
The Health and Wellness Coordinator will also work closely with the Anglicare at Home team to meet the social, wellbeing and health needs of our residents.
Organise a calendar of events and groups with a focus on wellbeing and social connectedness.
