The role supports residents in accessing care and support when they need it, with a focus on health, wellbeing, and social needs.Key responsibilities:Undertake health and wellbeing assessments for new and existing residents.Build rapport with residents to identify Home Care services that best meet their care needs.Support residents through the My Aged Care process to obtain assessments and referral codes to access home care services.
