About the role Agile Skills is seeking a dedicated Support Coordinator to join our team in Mackay, Queensland.
As a casual Support Coordinator, you will play a vital role in helping older adults and people with disabilities access the care and support services they need to live independently and participate fully in their communities.
What you'll be doing Assisting clients to navigate the aged care and disability support systems and access appropriate servicesDeveloping individualised support plans in collaboration with clients, their families, and other stakeholdersCoordinating and monitoring the delivery of support services, ensuring high-quality care and client satisfactionAdvocating on behalf of clients to ensure their needs are met and their rights are protectedMaintaining accurate records and documentation in line with organisational and regulatory requirementsCollaborating with a multidisciplinary team of healthcare professionals to provide holistic supportWhat we're looking for Relevant tertiary qualification in a health, social work, or human services-related fieldMinimum 1 years' experience in a similar support coordination or case management roleExtensive knowledge of the aged care and disability support systems, funding schemes, and available servicesStrong interpersonal and communication skills, with the ability to build rapport and work collaboratively with clients, families, and service providersExcellent organisational and time management skills, with the ability to prioritise and multitaskCompassionate and empathetic, with a genuine commitment to improving the lives of older adults and people with disabilitiesWhat we offer At Agile Skills, we are committed to providing a supportive and flexible working environment that empowers our employees to thrive.
