This project offers the opportunity to shape the future of healthcare delivery, aimed at improving patient care and outcomes.Job DescriptionJoin our innovative virtual care team, where you will be providing initial mental health assessments, triage, and support to individuals through a cutting-edge, fully virtual service.
This role offers the opportunity to be part of a groundbreaking new model of care, enabling timely interventions for mental health needs.
You will work closely with a multidisciplinary team to ensure clients receive appropriate, high-quality care.Key Responsibilities:Conduct virtual mental health assessments, prioritize cases based on urgency, and ensure timely interventions.Coordinate with a range of healthcare professionals to develop and implement care plans for clients.Provide emotional support, mental health education, and resources during the triage process.Refer clients to appropriate mental health services such as psychologists, social workers, or community resources.Maintain accurate and confidential electronic health records and contribute to reporting and evaluation processes.Qualifications:Bachelor's degree in nursing and current AHPRA registration.
Experience: Minimum of 5 years in mental health nursing, with a strong background in intake triage or crisis intervention.Skills: Proficiency in virtual care delivery, telehealth platforms, and electronic health record systems.
