About the Role The Casey Social Work Department is looking for a Grade 1 social worker to join the team working in the subacute hospital setting including Gem and Rehab programs.
This position is a full-time, permanent position and is part of a flexible team who collaborate to provide a holistic approach to service delivery across Casey Hospital, also working collaboratively across all sites bed-based and community.
Showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psychosocial assessments.Development of patient centred care plans and complex discharges.Bed-based service delivery includes rosters servicing the Emergency Department, and experience in acute and women's and children's wards.You will be a member of a dynamic Social Work and Allied Health team working in a collaborative model of care across Monash Health.Work closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.A Day in the Life: As a Monash Health employee, you will have the ability to maintain a high level of communication with patients, visitors and colleagues enabling you to deliver exceptional care.

The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support.
