We strive to lead the change in the way Australians think about ageing, recognising 'old age' as a positive force in society and giving a voice to those who have seen it all and live to tell the tale.We are seeking an experienced and motivated Case Manager to join our Home Care team in the Central South Metro region to service areas south of the river from Hamilton Hill to Kwinana and suburbs in between. Based at our Port Kennedy Home Care office, this role will involve travel to client's homes.This role is being offered on a permanent full-time basis working 38 hours Monday – Friday, with flexible working arrangements available. This position is suited to a candidate with a strong background in Case Management or Allied Health / Registered Nurse experience in the community.About the Role:Oversee and coordinate the onboarding process of new Home Care Package clients for your region.Develop ongoing relationships with local networks and external agencies to strengthen access to services and supports for daily living that are important for the client's health.Assist clients in identifying ongoing needs and goals to support them to remain in their own home.Responsible for ensuring client budgets are monitored and managed in line with approved package level and individual client requirements.Be an effective member of the multi-disciplinary team to ensure all decisions and referrals for clients deliver a high standard of holistic care.Maintain reports, information and support as requested by the Regional Home Care Manager to a high quality and in accordance to policy and procedures.About You:Experience in a homecare case management environment either with a clinical background or more than 3 years as a Case Manager in a similar organisation.Knowledge of Case Management principles, Home Care Package Guidelines and Funding body.Experience developing and managing client budgets.Ability to work within a team, as well as autonomously.Excellent organisational skills and ability to manage multiple priorities.Excellent verbal and written communication skills and strong computer skills and experience with client database systems.Valid driver's licence and access to a reliable vehicle.Current National Police Clearance, or willingness to apply for one.Willing to obtain required vaccinations – COVID 19 (minimum of 3 doses) and annual flu vaccination.Working with Bethanie:At Bethanie we are committed to being an employer of choice by creating a supportive and compassionate environment for our people and customers.
