We are part of WA's transformative reforms in Out-of-Home Care (OOHC), which aim to create a culturally sensitive and responsive system.
Key priorities include promoting stability for children in care, strengthening cultural connections, supporting early reunification, preparing young people for transitions, and delivering flexible, needs-driven services.About the ProgramThis program focuses on creating therapeutic environments for children who have experienced trauma, abuse, or family violence.
By promoting healing, stability, and cultural connection, we aim to improve outcomes through holistic, evidence-based interventions and collaborative care.About the RoleAs a Care Coordinator, you'll support children and young people in the OOHC program by:Collaborating with carers, families, and service providers.Developing and implementing individual care plans.Managing cases using a trauma-informed approach.Facilitating external referrals as needed.Undertaking some weekend events and after-hours roster work.Key RequirementsWe're looking for someone with:A tertiary qualification in Psychology, Social Work, or a related field.Experience in case management and working within the OOHC sector.Strong knowledge of trauma and attachment frameworks.Exceptional interpersonal and decision-making skills.A current driver's licence and Working with Children Check (or equivalent).Why Join Us?Competitive remuneration and a company vehicle.Tailored salary packaging and access to wellbeing programs.Professional development opportunities, including our Online Resources Library.Apply NowJoin us in making a difference.
