Mosaic Multicultural Connections – Charlestown NSW Mosaic Multicultural Connections is a not-for-profit organisation supporting culturally and linguistically diverse (CALD) individuals and communities across the Hunter, Central Coast and New England regions. We deliver person-centred aged care services that enable older people to live safely, independently, and with dignity—while maintaining their cultural identity. About the Role As a Care Coordinator, you will be responsible for providing culturally safe, responsive, and tailored care management to older people living at home. You will work collaboratively with participants, carers, health professionals, and service providers to deliver holistic, wellness-focused support. Key responsibilities include: Developing person-centred care plans aligned with participant goals Coordinating services to ensure continuity and quality of care Monitoring, reviewing and updating care plans as needs change Managing risks and escalating concerns appropriately Maintaining accurate documentation and ensuring compliance with Aged Care Quality Standards Monitoring participant budgets, forecasting services, and reconciling invoicese Educating participants and families about aged care rights, options, and supports Contributing to continuous improvement and service development within Mosaic About You We’re looking for someone who is compassionate, organised, and committed to culturally safe practice. You will bring strong experience in aged care or disability case management, with the ability to build trusting relationships and deliver high-quality support. Diploma qualification in Community Services, Aged Care, Case Management, or equivalent experience Experience in case-managed community aged care or disability services Demonstrated experience in care coordination or case management Strong understanding of the Support at Home Program and current aged care reforms Knowledge of aged care rights, Quality Standards, and trauma-informed practice Excellent communication skills with high cultural competence Proficiency in Microsoft Office and care management systems (e.g., Sandwai) Experience supporting people with dementia, palliative, or complex needs Training in cultural safety, restorative practice, or trauma-informed care Familiarity with My Aged Care portals or digital case management tools Why Join Mosaic?
