Where you'll be working Hospital in the Home is based across three sites in our NSLHD including Royal North Shore Hospital, Mona Vale Hospital and Hornsby Hospital. What You'll Be Doing Delivering acute care to patients in their own homes, along with other members of a multidisciplinary team. Key areas of practice include pressure care, falls prevention, new home oxygen prescription, post cardiac and post orthopedic procedures that impact on a person's occupational performance and function. People of Aboriginal and/or Torres Strait Islander background are encouraged to apply. Candidates Will Need To Meet The Following Criteria Must hold registration with the Occupational Therapy Board of Australia. NSW Drivers Licence. Demonstrated well developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders. Demonstrated computer literacy and ability to use health information systems. Demonstrate skills and knowledge in the provision of Occupational Therapy service within a Hospital in the home model of care to consumers, carers and families requiring assessment, environmental modifications, home modifications, appropriate referral and discharge planning.
