Company Description COSY Coordination is dedicated to providing comprehensive NDIS Capacity Building services all under one roof, including Support Coordination, Psychosocial Recovery, and Allied Health Services such as Counselling. We also offer Art Therapy and other skill building services to ensure an integrated approach to care. Based in the Greater Geelong Region, COSY is at the forefront of innovation in the Disability Industry, delivering high-quality support to empower individuals in achieving their goals. Role Description This on-site role, based in Geelong, VIC, is for an NDIS Support Coordinator. The Support Coordinator will assist individuals in navigating the NDIS framework, connecting participants with appropriate supports and services, and ensuring that their plans are effectively implemented to enhance capacity-building and independence. Responsibilities include liaising with various service providers, monitoring participant progress, and providing tailored support to meet individual goals. Qualifications Strong interpersonal skills and a commitment to fostering positive relationships Excellent verbal and written communication abilities Experience in Case Management and Customer Service Proficiency in providing Technical Support and problem-solving Understanding of the NDIS framework and participant-focused care Ability to work effectively both independently and as part of a team Relevant qualifications in social work, community services, or a related field are preferred Previous experience in the disability sector or with NDIS participants is a plus #J-18808-Ljbffr
