Community Support Worker (Full-Time Max Term role until 30/06/2026) Want to make a difference? Join the New Horizons team as a Community Support Worker and work within our Housing & Accommodation Support Initiative Program in the Hurstville area. About the Role This role provides support and works in partnership with people living with disability to live within the community in the way that they want to. We strongly encourage people with a lived experience of mental‑ill health to apply for this position. We empower our customers by supporting them to build independence, identify and achieve goals, engage in activities of daily life, and encourage genuine connections within the community. Key Responsibilities Collaborating with customers to develop individualised person‑centered support plans that incorporate their identified wellbeing needs, strengths, and goals. Providing support to customers to improve their independence by assisting with skill development, transport, social activities, supporting with finances, attending appointments, cooking, medication and personal care, as well as other support as required. Supporting and encouraging our customers to participate in activities of their choice, such as social, leisure, sporting, educational, employment activities etc. Keeping customer records up to date on a daily basis using online systems and electronic databases. Developing and maintaining professional relationships and networks with customers, families, caregivers and key stakeholders. Supporting customers to navigate the sector to access any additional service they may require. What our customers want from you Punctual Friendly Outgoing Non‑judgemental Flexible Guided by lived‑experience Persistent Balanced Empathetic Practical What you will bring to the role Minimum Cert III in Disability, Mental Health, Peer Work, or equivalent Valid NSW Driver's Licence (Class C), with ability to drive large cars and/or vans Willingness to obtain an Australian Police Check, NSW WWCC and NDISWC to meet New Horizons employment criteria prior to commencing with us. Great computer skills with proven ability to use Microsoft Office and customer relationships management systems.
