Our mission is to deliver improvements in the mental health, social wellbeing and economic participation of young people aged 12 to 25 years. The opportunity The Youth and Family Care Coordinator is an exciting role at headspace Strathpine that is responsible for coordinating the implementation of key parts of the young person’s care plan. This is a full-time, permanent opportunity within an established centre filled with a welcoming multidisciplinary team. As a Youth and Family Care Coordinator no two days will be the same as you will deliver psychosocial interventions (including skills development, improving self-care, and increasing social connection and engagement in structured activities) and ensure that there is effective coordination among the stakeholders supporting the young person. Responsibilities Collaboratively develop and manage care plans for young people to address their mental health and psychosocial needs. Coordinate and monitor the young person's care in collaboration with internal and external stakeholders. Provide individualised psychosocial support and interventions to young people, including skill development and increasing social connection. Incorporate family-inclusive practices and collaborate with family members and relevant individuals to support the young person's mental health. Maintain strong relationships with external stakeholders, conduct risk assessments, and ensure high-quality client records while complying with professional standards and legislative requirements. About You You’re passionate about supporting young people and skilled in coordinating care tailored to their mental health and well‑being. You thrive in diverse environments and excel at building meaningful connections with those around you. What you bring Tertiary qualifications and/or relevant experience in youth work, care coordination, or support work, with a focus on recovery and fostering independence. Expertise in youth mental health, including strengths‑based interventions, risk assessments, crisis response, and care plan development. Experience working effectively with multidisciplinary teams, engage families, and collaborate with external services and community networks. Strong organisational skills, autonomy, and attention to detail in documentation, including case notes and service coordination. Familiarity with mental health systems, policies, legislation, and referral pathways that support young people effectively.
