NDIS Support Worker Roles The Support Facilitator will be responsible for a broad range of tasks including 1-to-1 mentoring and support, working with clients in a group support capacity, and even the option for some day trips with clients. The role will report to the team leader and management team. This role is suited to someone who has a high degree of initiative and the ability to deal with a broad range of tasks. Great people skills are what we are looking for What's on offer: Casual or ongoing employment Immediate start (pending licensing requirements) Pay based on SCHADS award rate + super Positive and supportive work environment Key responsibilities for the Support Facilitator: Delivering one-to-one services including accompanying clients on errands and appointments One-to-one support including community access, emotional support and care, assistance with daily living activities Provide direct support services in accordance with individual care plans Assist with skill development for independent living, recreation, learning, and community access Advocate on behalf of the individuals and families to ensure services are relevant, flexible and accessible Maintaining client records with good note taking and document maintenance NDIS Support Coordinator roles The Support Coordinator's role is to facilitate growth in client capacity and ability to manage and engage with their NDIS services. Support coordinators support and empower their clients through knowledge, expertise, connection to their community, and assist them to make the most of their NDIS plan . The role will report to the Recovery Coordination Manager. This role is suited to someone who has a high degree of initiative and the ability to deal with a broad range of tasks. Experience in the NDIS as a support coordinator, or a solid understanding of how the NDIS functions is highly desirable but not required. What's on offer: Part time support coordination/ access support position moving to increase when possible. With working arrangements built around our mutual needs, including R.D.Os and other negotiable flexibility arrangements. Immediate start (pending licensing requirements). Pay above the SCHADS award rate + super (11%). Positive and supportive work environment Key responsibilities for the Support Coordinator: Provide Support Coordination services under NDIS Quality & Safeguards Commission guidelines and Reach Policy and Practice. Build strong and effective relationships with external provider organisations. Monitoring of NDIS plan budgets and support services to ensure plan budgets are utilised effectively and services are working towards the NDIS participant to achieve their plan goals. Liaise with participants and all relevant stakeholders. Promote a positive and professional image of Reach Social Supports. Achieve set targets, KPI's and caseload in a timely and effective manner.
