An exciting temporary position exists for an experienced Mental Health Clinician to work within the South Eastern Sydney Local Health District MHS Pathways to Community Living Initiative, helping mental health consumers on their journey to recovery.

The Pathways to Community Living Initiative (PCLI) is a state-wide strategy that is shaping complex care and community rehabilitation in mental health services in NSW. This initiative works with mental health services and community led organisations to develop pathways of care and services for people who have had or are at risk of long hospital stays and experience severe and persistent symptoms of mental illness. The program aims to ensure each person has the opportunity to live their lives in the community, to participate to the fullness of their lives and have access to well being, purposefulness and connectedness.

This strategy is led, funded and coordinated by the NSW Ministry of Health in collaboration with Local Health Districts. This is occurring as part of a decade-long whole of government enhancement of mental health care under the NSW Mental Health Reform 2014-2024.

What you'll be doing
The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional care, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it. Funded under the state-wide Pathways to Community Living Initiative (PCLI), this position is responsible for recovery-orientated, person-centred assessment, treatment and care for adults with serious and persistent mental illness and complex needs who have been in hospital for more than twelve months and/or are at risk of a protracted hospital stay to re-establish their lives in the community.

The PCLI Senior clinician will provide evidence-based care that supports safe transitions of adults into care in a community setting (or appropriate inpatient setting) based on the individual’s strengths, needs and wishes. The PCLI Senior Clinician will provide in-depth levels of clinical expertise and knowledge in complex care and rehabilitation in this process, they will plan, assess, implement and evaluate care in collaboration with the person and networks of people and services involved in their care.

These services are to be least restrictive, collaborative, nominated carers and family responsive and inclusive, rehabilitative, encouraging self-advocacy, facilitating empowerment and capacity for optimal independent function.

A primary goal of this position is to work collaboratively with the person, staff, carers and other agencies (both within SESLHD and government and non-government organisations) to promote effective participation in mental health treatment, support and recovery.

All Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant.

Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position.

Selection Criteria

  1. Social Worker or Occupational Therapist qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award Current full registration with AHPRA and/or eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer; Registered Nurse with current registration with AHPRA including relevant post-registration qualifications and at least 3 years experience working in the clinical area of their specified post-graduate qualification or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation; Recognised 4 year degree in Psychology, along with full registration with the Psychology Board of Australia (PsyBA).
  2. Extensive experience in mental health clinical and vocational rehabilitation with knowledge of best practice principles and models of practice; and ability to plan, implement and evaluate individual and group rehabilitation programs
  3. Demonstrated commitment to consumer participation, and a strengths based and recovery focused approach in service provision
  4. Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels
  5. Ability to provide in-depth advice, education and supervision to staff to enhance service excellence
  6. Ability to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel or complex nature
  7. Ability to initiate, lead, complete and share quality improvement and service evaluation processes
  8. Current Class C NSW Driver’s License
