Team Leader – Hospital in the Home Parkville Youth Mental Health and Wellbeing Service (PYMHWS) is a new, dedicated public youth mental health service and the first of its kind in Australia. The Team Leader will lead the delivery and development of this exciting model of care and will play a key role in ensuring high quality evidence‑based practices are delivered to young people in partnership with families and caregivers. You will lead the delivery and development of comprehensive acute and complex care to a diverse group of young people and their families. The role supports the team in the delivery of individually tailored services that comprise mental state assessment, risk assessment and management, discipline‑led interventions, medication, therapeutic interventions (CBT plus other targeted interventions), family inclusive practices, vocational and educational support, intensive outreach and crisis support. This role co‑ordinates bed flow and is central to liaison within PYMHWS for referral to the service alongside other key external stakeholders. What You’ll Do Provide leadership, fostering and demonstrating the specific values and perspective within the multidisciplinary HiTH team. Collaborate on the development of the HiTH model regarding care team planning, consultation and service delivery. Manage the daily operations of the HiTH team, including liaising with key referrers, shift planning, intake processes, escalation processes, staff orientation and well‑being. Ensure clinical processes are effectively managed, including allocation of clients, critical incident responses, clinical review, discharges, business and team meetings in consultation with the Continuing Care Team Coordinator and the Clinical Stream Leader. Maintain high standards of clinical care that are congruent with PYMHWS aims – client focused, accessible, responsive and consistent with best practice. About You You will be experienced in, and passionate about, providing acute, developmentally appropriate, and evidence‑based mental health care to young people and their families in the community, including those requiring complex support through the Hospital in the Home program. Disciplines: RPN Grade 5, Occupational Therapist, Social Worker, Psychologist Grade 4. Relevant registration and qualifications (see position description for details). Minimum 7 years’ experience in tertiary and/or youth mental health and a demonstrated ability to lead a clinical team, supervise mental health clinicians, and function both autonomously and collaboratively with other clinical and organisational leaders.
