$81,503 - $93,145 + Super + Salary Packaging About Us The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region. The Staying Deadly Hub aims to provide comprehensive assessment treatment and assertive outreach support to clients presenting with moderate to severe mental health needs. Working in a unique multidisciplinary care team as an extension of the IUIH comprehensive model of care, along with integration with the Staying Deadly Hub teams to provide appropriate and responsive care to clients would otherwise not access support or require inpatient care. About the Role The role will provide coordination of supports to Aboriginal and Torres Strait Islander clients who experience complex social and mental health conditions and to work effectively with other social and health practitioners to identify and access appropriate services and supports to meet their needs. This position is embedded in the IUIH Goodna clinic and works collegially with Counsellors, Psychologists and other social health team members along with the Family Wellbeing Service ensuring optimal client outcomes. In this role you will Process new referrals to SHP Intake Register and Case Management Database via MMex Initial triage/assessment of referrals to aid allocation – via telephone or face-to-face as required Contribute to Weekly Intake Meeting with SHP and FWS practitioners in the respective clinic/s Manage a caseload of complex clients ensuring systematic contact and follow up with clients to ensure priorities and actions are met, in conjunction with other team members (e.g. psychologists, counsellors, team leaders) to enhance levels of support and case support planning Undertake comprehensive assessment of clients’ psychosocial needs in consultation with the client and work with the client to develop a strength focused plan that sets out priorities and actions, including defining support/interventions from other SHP, MATSICHS or external providers About You Qualifications and experience required: Current C Class Drivers Licence (QLD) Current Criminal History Check or willingness to obtain Current QLD Working with Children Check (Blue Card) Current CPR Certificate Minimum Diploma of Health Services/Social Services/Social Sciences or relevant qualificationi Desirable, but not mandatory Demonstrated experience working with Aboriginal and Torres Strait Islander people and/or communities What Can we Offer At IUIH ‘Our Collectivity is Our Strength’.
