In this pivotal role, you'll do more than deliver high-quality care—you’ll drive innovation and continuous improvement. You will: Reduce avoidable hospital admissions through early intervention and comprehensive care Prescribe medications and provide specialist referrals as part of holistic, resident-focused treatment plans Support wellness initiatives , including monitoring residents on wellness programs and following up in partnership with GPs Lead clinical research by collecting, analysing, and interpreting wellbeing program data Contribute to policy development and shape best-practice care models within the aged care sector What skills will you bring? We’re looking for an experienced, motivated practitioner ready to make a real impact.
