Specialist Support Coordinator

As a Specialist Support Coordinator, you’ll work closely with participants who have complex needs (Level 2 and 3 NDIS participants) — helping them navigate services, achieve their goals, and build capacity for independence. You’ll join a warm, collaborative, and professional team that values respect, empathy, and excellence. This is a role where you can make genuine, measurable change in people’s lives. Key responsibilities: Coordinate supports for complex NDIS participants with care, consistency, and integrity Build strong relationships with participants, families, carers, and providers Use a coaching and strengths-based approach to build capacity and confidence Manage budgets, reports, and service agreements with precision and transparency Facilitate Care Team meetings and liaise with health professionals and stakeholders Provide crisis response and advocacy as needed, ensuring no one falls through the cracks Degree in Social Work, Psychology, Occupational Therapy, Speech Therapy or related field Experience supporting complex NDIS participants in a coordination or allied health capacity Excellent communication and relationship-building skill Strong organisational and time management ability Understanding of NDIS frameworks, service delivery, and ethical practice A current NDIS Worker Screening Check, WWCC, and Driver’s Licence Why you’ll love working with us Supportive, inclusive and collaborative team culture Opportunities for ongoing professional development and mentoring Competitive pay structure based on experience and qualifications A chance to make real, meaningful impact in people’s lives 360 Health Management is an Equal Opportunity employer, and we are committed to providing a working environment that embraces and values diversity and inclusion. #J-18808-Ljbffr
