MENTAL HEALTH AND WELLBEING LOCAL (LOCAL) The Mental Health and Wellbeing Local is community-led and integrated through a partnership sharing arrangement, creating a responsive, flexible, and helpful service. In Greater Shepparton Strathbogie - Moira (GSSM), APMHA Healthcare, Wellways and Goulburn Valley Health are working together to offer an easy way to access care and support for people aged 26 years and over who are experiencing mental health concerns – including people with co‑occurring alcohol and drug support and care needs, and their family, carers, and supporters. The Role The Towards Change Team Integrated Care Nurse will be employed by APMHA and will have responsibility for supporting participants with both mental and physical health with an emphasis on physical health and wellbeing, supporting participants to access physical health care planning through close liaison with the participants named GP or medical practice including advocacy and coordination of chronic disease reviews and medication reviews. We understand people living with mental ill health are more likely to experience poorer physical health than the general population and are more likely to experience comorbidity of physical conditions. Being equally well for our participants means that their mental health and physical health are screened and supported together. The role sits in the Towards Change Team, which includes multi‑disciplinary clinicians, providing tailored mental health and wellbeing support to participants and their families / natural supports via their wellbeing plan, including those with co‑occurring substance use issues and physical health comorbidities. The Integrated Care Nurse will work with all staff in a collaborative approach to ensure effective understanding and management of physical health needs. The Integrated Care Nurse role is office based, located at our purpose built and modern facility in Shepparton. Our service does offer assertive outreach so in‑home and community visits will be required. Our team also supports the Greater Shepparton Strathbogie and Moira communities so in‑hours travel to our regional towns may be required. Remuneration Competitive remuneration, commensurate to training and experience. The Candidate Will Possess: Current AHPRA Registration for Mental Health Nurses, Psychologists and Occupational Therapists Registration with AASW for Social Workers Post graduate experience working in a mental health or AOD setting (minimum 6 months) Training and experience delivering evidence based therapy Demonstrated time management abilities, excellent communication (written and oral) and interpersonal skills Demonstrate a friendly, proactive approach and ability to work with competing demands and balance priorities with efficiency and professionalism Ability to work effectively independently and within a team You Will Need AHPRA Registration (RN Div 1) and compliance with AHPRA Professional Development requirements Minimum 3 years post graduate nursing experience National Police Check International Police (if required) Current Working with Children Check – employment Evidence of right to work in Australia NDIS Workers Screening CheckNDIS Workers Orientation Modules – free online course Refer to the Position Description for full details on required qualifications, experience and compliance.
