The role involves assessing, planning, implementing and evaluating care in accordance with the Australian Nursing and Midwifery Council (ANMC) National Competency Standards competencies and/or Australian Council of Midwives (ACM) guidelines, NSW Health and SESLHD policies. You will undertake NSW Health child growth and development assessments and maternal psychosocial assessments to identify family strengths and needs, provide health promotion, early childhood intervention, parenting support, and coordinate referrals and follow‑up care. Collaboration with early childhood educators, schools, GPs and community partners is central to the role. Where you'll be working Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District. PaCH provides community health services to individuals, families and carers, focusing on the whole community or key groups in the community. Services are provided at the district’s hospitals, community‑based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services. The Role SESLHD is committed to enabling the community to be healthy and well. The Child & Family Health Nurse will coordinate and deliver high‑quality nursing care to clients, consistent with SESLHD policies, procedures and standards. The scope includes: Assess, plan, organise, evaluate and deliver holistic quality nursing care in accordance with ANMC and ACM standards, NSW Health and SESLHD policies. Provide health promotion activities and Child and Family Health Nursing clinical services, including physical, emotional and psychosocial assessments to identify family strengths and vulnerabilities for early identification and referral to appropriate interventions and social supports. Work with early learning educators, principals, teaching staff, learning and support teams, school counsellors/psychologists, general practice and local health and interagency agencies to deliver services to priority communities. Contribute to care coordination of children and families by providing short‑term management, including active follow‑up for referrals. Qualifications Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia, and relevant postgraduate qualifications related to the specialty area of Child and Family Health. Demonstrated ability to work autonomously as well as in multidisciplinary and multiagency teams. Experience working in partnership with families delivering Child and Family Health Nursing Services within a primary health care setting. Experience working with priority populations, specifically culturally and linguistically diverse communities. High level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others. Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Registered Nurse in applying these principles. Basic computer skills and proven ability to use health information systems. Current unrestricted NSW driver’s licence with a willingness to travel as required.
