As a Home Care Partner with UPA, you will support our clients to remain independent in their own homes and stay connected to their local community. We are offering an exciting opportunity for a Care Partner to join our Hunter team based in Wallsend About the role The Home Care Partner works in partnership with clients and the care team to co-design care plans based on the client’s goals, preferences, and assessed needs. The role supports functional independence, wellness and lifestyles choices. You’ll also liaise with internal and external stakeholders to achieve positive client outcomes within resource and budget allocations.
