Job Overview The postholder will be a member of the Community Learning Disability team, working closely with the multidisciplinary team. Responsibilities include the maintaining of a clinical caseload and conducting assessments of highly complex problems which require the analysis, interpretation and comparison of a range of solutions for adults with learning disabilities and their families who are experiencing complex health difficulties, associated with their learning disability. This role will include the maintaining of clinical standards within the governance framework. Main duties of the job To work within their code of practice to take responsibility for their caseload, ensuring comprehensive assessments of their needs. This includes the formulation, planning, delivery and evaluation of care and will involve the regular review and updating of risk assessments, assessment documentation and care plan reviews. To work collaboratively across a wide range of multidisciplinary services and agencies to provide effective delivery of care, supporting a programme of equal access to mainstream services with reasonable adjustments with collaborative interventions from learning disability services. To engage with working towards service developments and embracing new initiatives, as well as supporting service outcome measures using clinical and service audit. To contribute to the development and implementation of learning disabilities strategic planning and service development within the Learning Disability Directorate. They will participate fully in the professional activities of Nursing within the Division, gain further experience of professional issues and be familiar with current strategic thinking in the NHS in general. Essential criteria Registered Learning Disability Nurse RNLD Experience of working within Learning Disability services. Knowledge of community-based working and the challenges related to same. Evidence of training in psycho-social interventions or relevant proven experience. Evidence of Continuous Professional Development. Knowledge of the Putting Things Right Process High level of nursing assessment / risk assessment and evaluation skills Care and Treatment Plan (CTP) working practices Working within the Mental Capacity Act and other relevant legislation Experience of working with Safeguarding concerns Evidence of working in a multi-Agency setting / team Evidence of Leadership skills Proven skills in motivating and influencing others Evidence of advocating for people with a learning disability Desirable criteria Undertaken student supervisor and assessor training Clinical Supervisor Evidence of skills in areas such as Working with families. CBT. Teaching Group Work Epilepsy Anger Management Anxiety Management Challenging Behaviour LD & ASD Level 3 Safeguarding ICT knowledge and skills Evidence of skills in areas such as Working with families. Experiance Experience of the recruitment process Experience of working with complex trauma Experience of the delivery of Learning Disability specific education and training.
