Case Coordinator - Aged Care Amplar Health is looking for a part‑time Case Coordinator (0.6 FTE) to support our Aged Care program. This role is based in Adelaide and can be performed from home. Responsibilities Conduct initial phone assessment, care planning and goal setting. Manage real‑time escalations from on‑road clinicians. Coordinate in‑home services for clients receiving home care. Build and maintain effective relationships with stakeholders. Monitor patient progress and update care plans. Liaise with healthcare professionals and specialists. Perform home visits to undertake assessments when required. Discharge planning including referrals to long‑term services. Provide telephone support, education, and advice. Qualifications Recent nursing experience supporting consumers in a community setting. Experience working within a multi‑disciplinary team and remotely. Experience supporting consumers in the community or aged care setting. Experience in the home care package setting is desirable. Excellent communication and customer service skills; ability to build rapport with consumers. Ability to work autonomously while contributing to a supportive team.
