Key Responsibilities Coordinate and monitor delivery of services under Home Care Packages (HCP) / Support at Home Support intake, onboarding for new clients Manage and support a team of community care workers Liaise with families, GPs, allied health professionals Ensure compliance and quality as per standards Optimise budgets and resources Qualifications Experience working in Home / Community Care as a Case Coordinator or Home Care Manager Experience in managing and coordinating Home Care Packages HCP / CHSP or NDIS Excellent engagement and communication skills Amazing customer service, friendly approach, compassion and values focused Current driver's licence National Police Clearance What you can expect in return $128,800 package + $16K Salary Pkg + Laptop + Phone Company Vehicle provided Housing - fully subsidised Build lasting relationships with clients and represent the company Amazing organisation with leaders who value your contribution Brilliant onboarding, orientation to ensure you feel at home Fantastic culture, very inclusive, friendly and excellent leadership Positive and progressive organisation with a values driven approach Immediate Interviews Contact Please call Karen Sharp, Recruitment Manager on or email : for more information.
