HammondCare Canberra, Australian Capital Territory, Australia Clinical Partner – Community Nurse (Aged Care Partner) HammondCare – Home Care is currently looking for an experienced Community Nurse – Aged Care Partner to support the team in Canberra. Essence of the role The Community Nurse – aged care, known internally as Clinical Partner, works within HammondCare’s Mission in Action to deliver high‑quality, person‑centred clinical care that supports clients to live well in their own homes. The role provides evidence‑based, best practice clinical care holistically, to individuals, ensuring their unique goals, needs and preferences are understood and addressed. This role is focused on assessing clinical care needs, developing and reviewing care plans and delivering excellent clinical care in partnership with clients and key stakeholders. The role works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high‑quality clinical care. Essential / Mandatory Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action. Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General). Passion for and previous experience in client‑focused service delivery in aged and community/home care settings. Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards. Excellent communication and collaboration skills, with a commitment to advocacy and person‑centred care. Ability to work effectively in a multidisciplinary team and independently across multiple sites. A current driver’s licence and the ability to travel between HammondCare’s sites and services. Excellent administrative and computer skills, including digital health records and care coordination tools. Commitment to professional development. Empathy, patience, and a passion for delivering high‑quality, person‑centred care. Desirable Nursing experience in aged care and community care. Ability to identify and implement innovations in care delivery and service improvement.
