Care Manager / Enrolled Nurse MyCareer Hub is recruiting on behalf of Care Services Australia (CSA) for an experienced and dedicated Care Manager / Enrolled Nurse to support clients under the Aged Care program. This role requires strong clinical knowledge, excellent coordination skills, and a commitment to delivering high-quality, compliant care. About the Role As a Care Manager / Enrolled Nurse, you will be responsible for the clinical oversight, care management, and service coordination of HCP clients, ensuring all plans, assessments, and documentation meet regulatory and organisational standards. The role includes liaising with families, service partners, suppliers, GPs, hospitals, and support workers to ensure seamless service delivery. Key Responsibilities Clinical Care & Governance Complete initial and ongoing clinical assessments and develop accurate care plans. Identify client risks or changes and elevate and escalation appropriately. Lead weekly clinical meetings and maintain all required documentation and Visual Care notes. Collaborate with allied health providers, GPs, hospitals, and partners. Care Management & Coordination Coordinate services, review rosters and timesheets, process service requests, and manage supplier onboarding. Approve invoices, monitor expenses, and manage client budgets. Maintain all compliance logs, progress notes, and updates in Visual Care. Liaise with internal auditors and ensure all required client documents are maintained. Support Worker Oversight Provide guidance, mentoring, and performance oversight for support workers. Ensure shifts are covered, punctuality is upheld, and client satisfaction is maintained. Conduct client check-ins and handle all service-related enquiries. Community Engagement & Partnerships Build and maintain relationships with partners, GPs, and community organisations. Attend community or networking events where required. Strengthen referral pathways and support organisational growth. What We’re Looking For Enrolled Nurse qualification (EN). Experience in Home Care Packages, care coordination, or clinical care (preferred). Strong understanding of Aged Care Quality Standards. High attention to detail, excellent communication skills, and strong time management. Confidence using care management systems (Visual Care desirable). #J-18808-Ljbffr
