Personal Care Worker - Hospital-to-Home Transitional Care (Gold Coast South) About the Role As aPersonal Care Worker in the community, you’ll work collaboratively with a team to support older clients in their homes. Our non-medical care model includes services necessary to help patients safely and smoothly transition from a healthcare facility like a hospital to their home.
