Job Security - Permanent Full-time Salary Sacrifice & 5 Weeks Annual Leave Tools of the Trade supplied Care Partner Warwick & Toowoomba | Permanent Full-Time Opportunity About the Role Churches of Christ is seeking a compassionate and dedicated individual to join our Southern/Darling Downs Home Care team as a Care Partner to be based at our Toowoomba and Warwick offices. The Toowoomba hub is centrally located on Ruthven St close to all amenities and our Warwick office is set in a modern, lightfilled office space surrounded by rolling country scenary and beautiful gardens. In this essential role, you'll be the trusted connection between older Australians and the new Support at Home program—ensuring they receive the right care, at the right time, in a way that respects their choices and supports their independence. As a Care Partner, you’ll lead by example, supporting a team of Home Care Workers to deliver outstanding, person-centered care. Your role will include building strong relationships with clients and families, coordinating services, and ensuring our clients’ needs are not just met—but exceeded. What You’ll Do as a Care Partner Understand and Assess Client Needs: Conduct initial and ongoing assessments to identify physical, emotional, and social support needs. Work alongside clients and their families to co-design personalised care plans. Plan and Tailor Support: Develop, review, and adjust care plans to align with client goals and funding under the Support at Home program. Coordinate Care Services: Organise in-home and external services to ensure timely, quality care delivery. Empower and Advocate: Help clients understand their funding, choices, and rights. Advocate for access to the services they need. Budget Guidance: Monitor individual client budgets and assist them in using their funding effectively. Build Meaningful Connections: Foster trusted relationships with clients, carers, and providers to ensure continuity of care and smooth service transitions. Uphold Safety and Compliance: Maintain accurate records, comply with aged care regulations, and proactively manage risks to client safety and wellbeing. Your consistency, empathy, and attention to detail will play a vital role in supporting older Australians to live well at home. Who We’re Looking For You’re someone who genuinely cares. A natural communicator, you work well in a fast-paced environment and enjoy making a difference. You’re confident in the aged care space, organised, and skilled in problem-solving. Our Ideal Care Partner Has excellent communication and time management skills Understands Home Care Packages, Support at Home, and Commonwealth Home Support Program Holds a Certificate III in Individual Support (or equivalent) Holds a Medication Competency Holds (or is willing to obtain) First Aid and CPR qualifications Holds (or is willing to obtain) a current Police Check Holds (or is willing to obtain) a National Personal Insolvency Check Has a current driver’s licence - A fleet car will be supplied for use during work hours Be reasonably tech-savvy - A phone and laptop will be supplied for work purposes Has valid Australian work rights (please note: we are unable to offer sponsorship at this time) Above all, you’ll be someone who aligns with our core values: Integrity, Compassion, Excellence & Courage Why Join Us?
