About the Role As a Care Partner youll play a vital role in delivering person-centred support to clients across the Cairns region in both the disability and aged care sectors. Youll coordinate services develop care plans and ensure our customers receive the right support at the right time promoting independence dignity and quality of life every single day. This is a permanent part-time position based in our Cairns office working Monday to Friday for 37.5 hours per week. What Were Looking For Experience in disability and aged care settings Strong problem-solving abilities you think on your feet Excellent time management youre organised and reliable A true team player collaborative supportive and positive Essential Requirements Current Drivers Licence and reliable vehicle First Aid & CPR Certificate NDIS Worker Screening Check Working With Children Check and Police Check Availability for on-call support Desirable Minimum Certificate IV in Aged Care Disability Community Services or similar Experience in care coordination or case management.
