Your Next Chapter Starts Here BRAMS is seeking a full-time Care Coordination Team Leader, based in Broome, WA, to provide leadership and operational management across BRAMS’ Aged Care and Disability Supports services. This is a senior role responsible for leading a multidisciplinary team to deliver high-quality, person-centred, and culturally responsive care that reflects BRAMS’ Model of Care, Quality Care Framework, and Consumer Voice Framework. Key responsibilities include: Leading, mentoring, and supporting the care coordination team to deliver high-quality, culturally safe services. Facilitating case reviews, team meetings, and reflective practice sessions to promote accountability and consistent standards. Overseeing assessment, care planning, coordination, and consumer engagement processes that align with individual goals and community needs. Ensuring compliance with the Strengthened Aged Care Quality Standards, NDIS Practice Standards, and relevant frameworks. Building strong, culturally responsive relationships with consumers, families, and carers. Strengthening partnerships with internal teams and external providers to improve integration and continuity of care. Driving workforce capability, continuous improvement, and quality outcomes through structured supervision and data-driven practice. Is This You? The successful candidate will hold a tertiary qualification in Social Work, Psychology, Community Services, or a related field, and bring substantial experience leading multidisciplinary teams within care coordination, case management, or allied health settings. A strong understanding of the Strengthened Aged Care Quality Standards and NDIS Practice Standards. Proven ability to implement Continuous Quality Improvement (CQI) processes, including audits, incident review, and improvement planning. Experience in data-driven reporting, supervision, and reflective practice. Strong leadership, communication, and analytical skills with a commitment to transparency and accountability. Financial literacy, including experience monitoring billable hours and cost recovery. Current Working with Children Check, National Police Clearance, and Driver’s Licence. You’ll also bring personal qualities that align with BRAMS’ values: Professionalism, integrity, and sound judgment in all interactions. A commitment to collaborative leadership and building a positive, respectful team culture. Adaptability and resilience in complex or high-demand environments. Experience within an Aboriginal Community Controlled Health Organisation or community health setting, along with an understanding of Aboriginal culture, values, and the social determinants of health, will be highly regarded.
