ABOUT THE ROLE This position plays a pivotal role in the delivery of the Aged Care Assessment Program (ACAP). The focus of the role is to conduct assessments to work out whether an older person is eligible for government‑subsidised aged care. Assessors visit the older person’s home or hospital to: Understand the person’s care needs, goals and preferences. Decide on the types of services they may be eligible for Create a goal‑oriented support plan based on their care needs and preferences. All assessors use the My Aged Care assessor portal to manage referrals, record comprehensive assessments, develop support plans, recommend services, record approvals for care, refer a person for services, and review and update central client records. We’re looking for candidates with: Tertiary Qualifications, registration with AHPRA or appropriate governing body, and experience in any of the following disciplines: Nursing (RN only), Occupational Therapy, Physiotherapy or Social Work. Current, up to date qualification through Commonwealth department of health “MAC Learning” as at 1st November 2025 – (please supply MAC Learning transcript with your application). A passion for understanding the individual needs of older people, respecting their choices, preferences and diagnosed conditions, and understanding the journeys of their life.
