An exciting opportunity exists for aCHSP Care Partnerto join our Community Care Team, based in Dapto NSW. About the role This position provides an important client facing and referral role conducting assessments and completing support plans to register new Support at home and CHSP clients in a timely manner and in consultation with the Scheduling Officer and Services Team. This is a permanent position reporting to the Aged Care Senior Manager, working 18-20hours per week with days and times negotiable with the right candidate. The successful candidate To be considered you must have: Certificate III or IV in Home and Community Care and/or Disability or equivalent qualification or 5 years relevant experience One year experience in a similar position Experience in a supervisory or team leader position Experience in completing CHSP & HCP assessments Accurate and high level of administrative skills with intermediate level of computer proficiency, including use of Microsoft Office Suites and excellent data entry skills High level of attention to detail and excellent written and verbal communication skills Smart phone and reliable vehicle.
