About the program We have an opportunity for a Case Manager to join our Reconnect Team within Youth team. This role is a community-based earlyintervention and prevention program for young people aged 12 to 18 years whoare homeless or at risk of homelessness, and their families. The role willprovide culturally appropriate counselling, mediation and case managementthrough proactive community outreach. The Role The Reconnect Case Manager Position will entail undertakingthe following: Provide guidance and encouragement to young people at-risk or currently incontact with the justice system to achieve goals as detailed in their actionplan. Encourage positive relationships and support networks through role modelling. Educate, encourage, and support young people to engage with internal andexternal support services. Support Youth clients to identify short and long-term goals. Establish and maintain collaborative and supportive relationships with youthclients, service providers, other team members and the local community throughdemonstrating respect, empathy, and the willingness to understand differencesin culture and customs. Experience working successfully with at risk young people experiencing systemicdisadvantage as well as young people with justice involvement. Extensive knowledge of barriers facing young people in metropolitan Perth. Extensive experience working with at risk young people. Understanding of the issues faced by young people in Western Australia. Understanding of homelessness, drug and alcohol dependency, violence andchallenging family dynamics of young people. Knowledge and understanding of Aboriginal and Torres Strait Islander culturalcustoms, and protocols, and the diversity of issues affecting Aboriginal andTorres Strait Islander people in Western Australia, including rural and remoteareas. Experience and confidence in delivering mentoring, coaching and modelling toyoung people. Experience in establishing and maintaining positive and collaborativerelationships with partner organisations and other stakeholders. The ability to communicate sensitively and effectively and work well with youngpeople and their families. Other Employment Requirements Australian Drivers licence Working With Children Check (WWCC) Police Clearance (within 3 months) – A criminal record does not make youineligible to fill this position. Persons under current correctional supervision will not be considered for thisrole at this point in time.
