Support Coordinator (Level 3) – Full Time MDSQ t/a ndis support providers – Fairfield NSW We are seeking a dedicated Level 3 Support Coordinator to join our team in Fairfield. This is a full‑time role supporting a shared caseload of 56 NDIS participants, working alongside another experienced Support Coordinator. About the Role As a Level 3 Support Coordinator, you will provide high‑level, complex coordination supports to help participants implement their NDIS plans and achieve meaningful outcomes. You will work collaboratively with participants, families, service providers, and community supports to ensure effective and timely service delivery. Key Responsibilities Coordinate supports for participants with complex needs (Level 3) Develop, monitor, and review participant goals and support plans Build strong relationships with participants, families, and providers Resolve barriers to service access and ensure continuity of supports Assist participants to navigate the NDIS, mainstream, and community systems Maintain accurate documentation and progress notes Work collaboratively with the team to manage a shared caseload Requirements Strong understanding of the NDIS, support coordination, and complex case management Ability to work autonomously and manage competing priorities Excellent communication and problem‑solving skills Valid driver’s licence and reliable car Apply Now If you're ready to take on a rewarding role supporting participants with complex needs, we invite you to apply and start immediately. #J-18808-Ljbffr
