Help older adults stay active, independent and safe in the comfort of their own homes. The role is a new position with scope for the use of hybrid models of care delivery. You will work closely with members of the Transitional Aged Care Program (TACP) team, Allied Health Team Leader and TACP Manager. The Aged Care Division delivers a range of clinical and non‑clinical services across the entire Local Health District to support older people in their families to remain supported in their homes. Key responsibilities: Provide physiotherapy assessment, treatment and re‑ablement to older adults in community settings. Use hybrid models of care delivery (home visits, virtual consultations) to support patient outcomes. Collaborate closely with the TACP team, Allied Health Team Leader and TACP Manager to coordinate integrated care plans. Advocate for restorative care and promote independence for program participants. Respond to changing aged care demands, maintain high quality clinical practice and contribute to continuous improvement.
