About the role The Community Health Nurse (CHN) will provide case coordination and service navigation to residents impacted by the public housing high rise redevelopment. The role ensures residents with complex needs or face barriers accessing care are supported to transition safely to new living environment. The role is part of a multidisciplinary team providing wrap around support to impacted residents. Please refer to the Position Description for more information.
