Lead Practitioner, Specialist Older Adults' Mental Health Service

An exciting opportunity has arisen for a Lead Practitioner to join the Specialist Older Adults' Mental Health Service (based at Brighton General Hospital). We are a friendly and welcoming multi-disciplinary team, based in the culturally diverse and creative city of Brighton and Hove. This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 5-6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details). Main duties of the job We are looking for a practitioner with a background in Nursing, Occupational Therapy, or Social Work, with experience of working with older people with dementia and other mental health conditions. As part of the role you will be offering your professional input into the multi-disciplinary team, by carrying out initial assessments as well as holding a caseload. As part of the team structure you will receive professional supervision and be able to access regular CPD sessions and training. The successful applicant will develop their skills in carrying out assessments, formulating plans and managing a diverse caseload of service users, while maintaining high standards of documentation. The service is looking to appoint somebody who is highly motivated, enthusiastic, creative and resourceful, and who has a passion for working with older people and for developing good working relationships with carers. You will have the opportunity to forge close links with the third (voluntary) sector, as well as with other specialist teams within the Trust. About us The service operates Monday to Friday, from 9am to 5pm. The role will involve working from the team base, and across Brighton & Hove as required, with some flexibility around home working, dependent on the needs of the service.
