The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region. The focus of the IUIH Disability support coordination team is to provide well-coordinated, quality disability services to eligible clients by walking alongside them in utilising their NDIS plan, engaging appropriate NDIS services and advocating with the participant to ensure culturally appropriate and safe supports. IUIH is expanding existing and developing NDIS programs designed to support Aboriginal and Torres Strait Islander children and families to access disability services that meet their needs. About the Role? The position is responsible for independently assisting NDIS eligible participants in interpreting their NDIS plan, assisting them to establish the supports in their plan, managing their budget and developing their capacity over time to coordinate and manage their own supports. Your day-to-day duties include: Use your skills to assist complex and vulnerable members of the community to identify goals to address their NDIS needs and overall health and well being Conducting needs assessments based on each participant NDIS plan and goals Develop a support plan that identifies clear goal outcomes Working alongside a number of crucial stakeholders Negotiation between services to generate effective support plans Support participants to achieve their goals by building their capacity to coordinate their own supports Link to mainstream or community services (i.e. housing, education, transport, health) About You? Current C Class Drivers Licence (QLD) Current Criminal History Check - willing to obtain before commencement Current Working with Children Check (Blue Card) - required before commencement Current Disability Worker Screening Worker (Yellow Card) - required before commencement First Aid and CPR Ability to travel across the designated service region as required Experience working with a diverse range of people including people living with a disability and their families/carers. Capabilities in time management, prioritisation and the ability to manage a fast-paced caseload. Desirable, but not mandatory: Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities Demonstrated experience in the support coordination and/or NDIS field.
